Here’s Your Opportunity to Improve Community Health Care

Please help the Clinton County Health Department learn more about the health concerns of County residents and their families. Our goal is to collect information from residents over the next two months, but we need your help to reach as many people as possible with our brief survey. The information will be used to identify priorities for the 2017-2020 Community Health Improvement Plan.
The survey consists of twelve questions and generally takes less than five minutes to complete.
