ELIGIBILITY:
Are you 65 or older and a NYS resident
Income eligibility Single: up to $75,000 & $100,00 Married
Are you enrolled in Medicare Part D?
If you answer YES to these three steps, you may be eligible for the following:

Coverage Assistance In the “Doughnut Hole”
Low Cost Premium and Copay Assistance
No Cost Deductible Plan
